About the Symphony Post Acute Guest Satisfaction Survey
At SYMPHONY we are focused on providing our patients with an exceptional experience during each and every visit. To ensure we are meeting the needs of our guests, we contact them post-discharge to complete a survey to collect feedback on their stay and treatment with us. SYMPHONY utilizes a third-party call center, not affiliated with the network to collect this data. National Research Corporation receives this data and provides the technology used to display ratings and comments on our member facility profile pages so that prospective guests and their families may see what SYMPHONY guests have to say about their overall experience.
Who receives the survey?
The survey is offered via phone to all SYMPHONY guests within a few days following their discharge from a member facility. Guests are asked to provide comments regarding specific aspects of service and care. Results are used to evaluate guests' overall perception of service and care, and to identify areas for improvement. We use this feedback as a way to improve and enhance the care and services we provide.
Do you post all comments?
SYMPHONY is committed to transparency. All comments are posted, with these few exceptions:
Offensive, slanderous, libelous or profane language; individual's names or detailed descriptions that may jeopardize guest privacy/confidentiality; comments about other providers, departments or instances of care are not posted.
What questions do we ask?
We calculate our star ratings based on responses to the following survey questions:
- On a scale from 0 to 10, where 0 is the absolute worst and 10 is the absolute best, how likely are you to recommend the SYMPHONY facility to a friend or colleague?
- If you find yourself in need of skilled nursing or rehabilitation therapy again, how likely are you to return to this SYMPHONY facility? Very Likely, Possibly, or Not Likely?
- Comments/Suggestions
Why do we ask?
We want to make sure that SYMPHONY remains the best provider of skilled nursing and rehabilitation services and the provider of choice for all. We take the feedback from our guests and their families seriously, and want them to know that this feedback is valued and used for continuous improvement.
How is patient information protected?
SYMPHONY remains HIPAA compliant and guest or family names are never used or displayed with any online rating or comment/suggestion. All personal identification is removed prior to display. Only the comments/suggestions remain.
